Position
A Home Care Nurse provides skilled nursing care, support, and education to patients in their homes. They work with patients of all ages who require medical assistance due to illness, injury, chronic conditions, or recovery after hospitalization. The role involves monitoring health status, administering treatments, promoting independence, and collaborating with families and healthcare providers to ensure quality patient-centered care.

Responsibilities

1. Provide direct patient care in the home setting, including wound care, medication administration, injections, IV therapy, and other nursing procedures.

2. Monitor and assess patient conditions, recording vital signs and symptoms to detect changes in health status.

3. Develop, implement, and evaluate individualized care plans in collaboration with physicians and other healthcare professionals.

4. Educate patients and families on disease management, medication adherence, nutrition, hygiene, and lifestyle modifications.

5. Assist with mobility, personal care, and daily living activities when necessary.

7. Maintain accurate, timely, and confidential patient records and reports.

8. Respond to medical emergencies and take appropriate action to ensure patient safety.

9. Coordinate with multidisciplinary teams to provide holistic care.

10. Offer emotional support and counseling to patients and families to ease anxiety and improve well-being.

11. Uphold infection control protocols, patient safety standards, and ethical nursing practices.

Qualifications & Skills

i. Registered Nurse (RN) license (or equivalent,)
ii. Proven experience in home healthcare, hospital, or clinical nursing preferred.
iii. Strong clinical assessment and critical-thinking skills.
iv. Excellent communication and interpersonal abilities.
v. Compassion, patience, and the ability to build trust with patients and families.
vi. Ability to work independently with minimal supervision.
vii. Good organizational and time-management skills.

Working fair

1. Patient homes, community settings, and occasional clinical facilities. 2. Flexible hours, including evenings, weekends, and holidays, depending on patient needs.
